Upload and Submit Nominations

1. Please ensure that your nomination form is saved to your desktop and named properly using the format (L-1_lastname_firstname.doc)
2. Click the Browse button to find the saved nomination form on your desktop, select the word document and click Open.
3. Once the form name appears in the text box, press the Upload button.
4. You will have to repeat this process for each nomination.
5. (Please be patient as it may take some time for the form to upload.)
