This is our 5th year recognizing service improvement through the Service Breakthrough Awards. We have amended the process to include HCAHPS. There are 8 categories in Patient Experience (HCAHPS) and 8 categories in Patient Satisfaction.
The 2011 Service Breakthrough Awards Ceremonies and Luncheon will take place on Tuesday, January 17, 2012 in Dallas, TX at the 12th Annual HealthCare Service Excellence Conference.

- Submit each nomination form seperately using the Upload Page; or,
